Chloral hydrate와 Hydroxyzine 복합투여와 Benzodiazepines의 진정효과에 관한 비교연구

Abstract
결론
경희대학교 치과대학 부속치과병원 소아치과에 내원한 환자중 비협조적 행동양상을 보이
는 환자중 25-52개월(평균 35개월)의 어린이로 3회 이상의 치료가 필요한 22명을 대상으로
하였다. 내원시 마다 chloral hydrate(50㎎/kg)과 hydroxyzine(25㎎)을 복합투여 하거나,
midazolam(0.5㎎/㎏) 및 triazolam(0.25㎎)을 단독투여 하되, 약물의 순서에 관계없이 경구투
여한 후 진정효과를 평가하여 다음과 같은 결과를 얻었다.
1. Chloral hydrate와 hydroxyzine의 복합투여군과 midazolam 단독투여군에서 Houpt의
전반적행동 평가중 임상적으로 진정효과가 좋았다고 생각되는 “양호” 와 “매우양호”는
90.9%(20명)이었으며, triazolam 단독투여군에서는 77.3%(17명)이었다.
2. Chloral hydrate와 hydroxyzine 복합투여군이 midazolam이나 triasolam 보다 수면 효과
가 좋았다(P<0.05).
3. 울음, 움직임, 전반적 행동에 있어서 모든 군에서 유의한 차이를 보이지 않았다
(P>0.05).
4. Chloral hydrate와 hydroxyzine 복합투여군에서 2명에서 시술후 구토가 있었으며 호흡
억제 및 다른 부작용은 없었고 대체로 안정된 심박동과 동맥혈 산소포화도가 관찰되었다.
#초록#
The purpose of this study was to compare the clinical sedation effect of chloral
hydrate and hydroxyzine combination, midazolam, triazolam when young children were
sedated for dental treatment. The uncooperative 22 children aged, 25 to 52 months of
age (ASA class I) and weighting between Ⅱ and 17kg, participated in the study. Each
patient was assigned randomly to receive chloral hydrate (50mg/kg) and hydroxyzine
HCI(25mg), midazolam(0.5mg/kg). and triazolam(0.25mg) oral administration : alternative
regimens were administered at next appointment.
According to rating scale, sleep, crying, movement. and overall behavior response were
checked for evaluation for the clinical sedation effect. Pulse rate and oxygen saturation
were also measured for monitoring the patients during treatment period by pulse
oximeter.
The results were as follows :
1. In the evaluation of sedation effect, 90.9% in chloral hydrate and hydroxyzine
combination and midazolam, and 77.3% in triazolam were rated “good” or “very good
”.
2. Sleep was demonstrated to be statistically significant increase in chloral hydrate
and hydroxyzine combination group. Despite the fact that chloral hydrate and
hydroxyzine group was in a deeper state of sleep, all the children were easily aroused.
3. There were no statistically significance among the three regimens with regard to
cryins, movement, overall behavior.
4. The adverse side effect was vomiting in one patient of chloral hydrate and
hydroxyzine combination. There were no clinical sign of respiratory depression.
